Summer Enrollment Request Form

Thank you for your interest in Brookview's Summer Programs. See the list of Elementary & Primary Themes below. Our instructors will spend the mornings focusing on the weekly theme. Afternoons will involve doing crafts, spending time outside, and generally having summer fun! Please note that Infant and Toddler Summer Programs are only available to students who are enrolled for the School Year.

Because so many of our families travel during the summer months, registration is done on a week-by-week basis, and you will only be charged for the weeks your child is enrolled. You must check the box for each week and length of day that you would like your child to attend. Short weeks (Weeks of July 1 and Aug 19) will prorated accordingly.

With the exception of 3-day Infant and Toddler Programs, all programs are 5-day programs, with no refunds given for days your child does not attend.

Payment is due upon signing the Summer Enrollment Agreement. Current families may roll payments into their FACTS account.

For Infant/Toddler 3-day Programs, please choose the three days of attendance. We ask that these days be consecutive:

Week by Week Registration

You will only be charged for the weeks your child is enrolled. Changes are allowed until June 3, after which each change will incur a $25.00 charge.

The June 3-7 option is for Infant and Toddler Programs Only. Primary and Elementary Summer Camps begin on June 10.

Half Day is 8-11:20 for Infant/Toddler/Primary. 8-11:45 for Elementary.

School Day is 8-3:20 for Infant/Toddler/Elementary. 8-3 for Primary.

Extended Day is 8-5:20 for All.

Short weeks will be prorated accordingly.
